PPO: costs more, moves freely

A broad, usually nationwide network. No referral needed. Some out-of-network coverage. This is what you want if you travel, split time between states, or want to keep a specific specialist.

EPO and POS: the middle

EPO is a PPO network without out-of-network coverage. POS is an HMO that lets you go out of network at a higher cost. Both are compromises; both are less common.
